How to Use the Patient Portal

Our practice uses Patient Fusion’s patient portal. During your first visit, we will provide you with printed instructions to help create your online patient portal account.

How to Create an Account to Access the Patient Portal

If you don’t have a Patient Fusion patient portal account, you’ll need to create one to communicate with your provider and access your medical records online.

Please read this carefully before creating your account.  Patient Fusion verifies your identity by matching the information you provide when creating your patient portal account with the information that our office has on file.  You will need the following information that MUST EXACTLY MATCH the information our office has on file: first name, last name, date of birth, email address, and mobile phone number.


To create your Patient Fusion patient portal account, follow the steps outlined below:


1. Check your email.  Instructions to access the patient portal will be emailed to the email address that we have on file. Please note this email MUST BE A VALID EMAIL ADDRESS. Please check junk or spam folders. The email typically takes about 5 minutes to appear in your inbox. Open this email, and click “Access Your Records” .

2. A new web browser window will open up. Verify your identify by selecting either “Enter PIN received from the doctor” or “Request security code via phone”. The PIN number is the temporary PIN listed on the printed instructions you received during your visit. If you select, “Request security code via phone”, the phone number you enter on the next screen MUST EXACTLY MATCH THE MOBILE PHONE NUMBER we have on file.

3. If verifying identify using “Request security code via phone”, enter all of the following fields: “Patient’s date of birth (MM/DD/YYYY)” and “Your phone number from the doctor’s record” (this is the mobile phone number that we have on file). Select the method you would like to use to receive this security code: text or voice.

4. A temporary, 5-digit security code will be sent to the mobile phone number on file. Enter this code, and select “Check code”.


5. Select “Create a new account” or “Link to an existing Patient Fusion account”. If linking to an existing Patient Fusion account, you will need to know the email address and password of the patient account that you would like to link to. If you do not know this information, please contact our office at (425) 391-5270 for assistance.

6. To create a new account, you will need to complete all fields shown below and will need to create a password for this account. PLEASE RECORD YOUR PASSWORD AND ALL OTHER ENTRIES IN A SAFE PLACE FOR FUTURE USE. Passwords must have at least 8 characters, 1 uppercase letter, and 1 number or symbol. When all fields have been entered, click “Create new account”.

Your Patient Fusion patient portal account should now be created, and you’ll be able to communicate with your provider and access your medical records through this patient portal.
