How to Use the Patient Portal

Our practice uses Patient Fusion’s patient portal. During your first visit, we will provide you with a unique personal identification number (PIN) that Patient Fusion will use to verify your identity and printed instructions to activate your Patient Fusion account.

If you have already created a Patient Fusion account and have access to your medical records online, you can log in here:

How to Create an Account to Access the Patient Portal

If you don’t have a Patient Fusion account, you’ll need to create one to access your medical records online.  To create your Patient Fusion account, follow the steps outlined below:

 

1. After your appointment, check your email. You should have received an email from Patient Fusion that looks like this. Click the “Access your records” button.

 

2. Select “Enter PIN received from the doctor” and click the “Continue” button.

 

3. Enter the patient’s date of birth (format: MM/DD/YYYY including slashes) and the PIN you received during your visit. Then, click the “Verify identity” button.

 

4. Select “Create new account” and click the “Continue” button.

 

5. Enter your account details and click the “Create new account” button.

 

Your Patient Fusion account will be created, and you’ll be able to access your medical records through our patient portal.
